If applying mail the following:
- A completed Membership Application
- A signed copy of the Code of Ethics (keep one for your records)
- A copy of your dogs's current rabies certificates, receipt for city/county dog licenses, or your current kennel license
- Non-refundable $10 application fee payable to BMDCHR
- Optional newsletter fee
